How I use your information
Initial contact
When you contact me with an enquiry about my service I will collect information to help me satisfy your enquiry. This will include your name and also a method of contact such as an email address or phone number. Alternatively, your GP or other health professional may send me your details when making a referral or a parent or trusted individual may give me your details when making an enquiry on your behalf.
At the point of contact I will ask you if you would like your details to be added to my email mailing list. If you would like me to add you to my email mailing list, then I will store your email address on my computer. My email account and mobile phone are password protected and the computer/laptop that I use to respond to your emails, have anti-virus software and are password protected.
Unless you have requested a follow up call or conversation about my service I will delete your details from my phone and/or computer within one week of your initial point of contact.
Email Mailing List
I will use your email address to provide you with information about services, for example, upcoming classes, workshops, and ongoing groups. If you would like your information to be removed from the mailing list you can request this at any time by send me an email.
While you are accessing my services
I will use your email address and/or phone number to provide you with information about my service. Unless we have agreed beforehand, that I will use your email address or phone number to forward you additional information, I will only use your email address and phone number to contact you about the aspects of service for which you are signed up for.
When registering you for an event such as a workshop or an ongoing group I may ask you to complete a Personal Details Form containing your name, address, date of birth, contact information and also contact information for your GP. When away from the event itself, this form will be stored in a locked filing cabinet that can only be accessed by me and the cabinet is located in an office that is locked when I am not using it. The Personal Details Form will be confidentially destroyed within one month of the end of the workshop or ongoing group.
I am a Member of the 5Rhythms Teachers Association (5RTA) and I abide by the 5Rhythms Teacher Code of Ethics. I am also required to attend regular Teacher Refresh Training Events.
I will treat as confidential any information that is disclosed to me personally or discussed within the group, with the following exceptions: if I believe that your own life is at risk of serious harm; if you disclose that harm is being carried out to another person; if you disclose information about criminal activity such as money laundering or terrorism; or, if a court of law requires me to disclose such information. Unless there are safeguarding issues that prevent me from doing so, I will always aim to speak with you before needing to break confidentiality. In group settings I will encourage an atmosphere or mutual respect and confidentiality, but I cannot be held personally responsible for the actions of others.
For the purpose of assisting our work together I will sometimes keep developmental process notes. The notes are for my use only and they help me to reflect on my teaching. The notes will be stored securely in a locked filing cabinet that only I have access to and the cabinet is located in an office that is locked when I am not using it. At no point would I personally identify a specific individual in these notes. Notes are not disclosed to third parties with the following exception: in rare cases, courts of law can demand that process notes are handed over.
After you have attended a class, workshop, or ongoing group
If at any point you decide that you are no longer interested in being notified about my services, or if you wish to change the information that I hold about you, you can request that I delete and/or change the information that I hold, by sending me an email. Unless instructed from you otherwise, I will keep your email address for my own future marketing purposes.

Where I have contracted with a supplier to carry out tasks I have carefully selected which organisations I work with. I state that they do not use your information in any way other than the task for which they have been contracted. I will never pass on your contact details to any third party organisations for the purposes of sales, marketing or research.
If your attendance fee is paid for or arranged via a third party, for example, your employer, the only information shared with the third party for invoicing and payment purposes, is the dates of attendance and non-attendance. Details about what occurs in the classes, workshop and/or ongoing group, will remain confidential, within the limitations of confidentiality outlined above, and details will only be shared with third parties if you give me your written consent to do so.
